We serve kids ages newborn through 18 living in western Wisconsin.

Complete one form per child. Volunteers select items using the child’s sizes, preferences and fit hints.
We will do our best to meet the student’s needs, but cannot guarantee all requested items will be available.
We will contact you when items are ready to arrange pick-up, meet-up or drop-off.

Youth: Numbers only (for example 8, 10, 12).
